What is Remote Patient Monitoring?

Remote Patient Monitoring or “RPM” represents a recent advance in healthcare technology that allows doctors and patients to establish a stronger relationship and focus on a patient’s daily health condition. We can now check daily vital signs at any time and transfer those readings instantly to your providers so they can ensure vital signs are within appropriate ranges. The value of taking daily readings is that any changes or trends can easily be identified for your doctor to review and take action if necessary.

How do I know if I qualify?

All Medicare patients with one or more chronic conditions are eligible for full reimbursement, except for a nominal co-pay required by some practices. You can consult with your provider to determine your eligibility. Once approved, It only takes a few days to get set up.

Is it easy to take daily readings?

The LiveCare platform is extremely easy to use. Once your provider prescribes you for RPM services, a LiveCare Link+device will be assigned and you can start taking your daily vital readings at home.

What vital signs will be monitored?

You will receive three basic devices to start monitoring your oxygen level, pulse rate, weight, and blood pressure. Your provider will set acceptable ranges based on your readings. If your readings fall outside these ranges, the provider will contact you to determine why this may be happening, and if any corrective actions are necessary to address the conditions.

What is a Pulse Oximeter?

The Pulse Oximeter device measures the oxygen level in your blood and your pulse rate. Changes in oxygen levels are strong signs that something more serious could be affecting your health, especially during the Covid-19pandemic. Consistent pulse rates are also important signs of a good heart.

Why is monitoring weight important?

One of the devices you will receive is a digital scale. It is important to maintain a healthy weight to lower the risk of heart disease, strokes diabetes, and high blood pressure. It can also lower the risk of many different cancers. We monitor your change in weight over time as this is a good way to monitor the status of your heart. Short-term weight gains could be a sign your heart isn’t working right.

Why do I need to monitor my blood pressure regularly?

Monitoring Blood Pressure is key to your health maintenance regimen. In order to determine if medications are working, check your blood pressure on a regular basis. Our system makes this an easy process with a simple arm cuff and automated device. All of your readings will be sent and automatically uploaded to your provider for review.

The platform also has a life alert system.

As an added benefit to the LiveCareplatform is a built-in Life Alert 24×7 connection to an emergency response call center. By simply pressing the emergency alert button on the Link+device, an emergency responder will contact you within 60 seconds of placing the alert. This service can travel with you anywhere you take the Link+ device. This service alone can cost up to $30/month and is included free with our RPM service.

The Link+ device has telehealth.

The Link+ device transmits your vital signs and it can also serve as a video conference platform with your provider. So now your provider can check your vitals remotely and conduct a remote visual inspection using the Link+ video capability.
